Are you, or do you know, a top-notch benefits adviser under 35? Nominate your Rising Star today for possible inclusion in EBA's upcoming magazine. The deadline to submit is Monday, February 17, 2014.

Employee Benefit Adviser’s Rising Stars under 35

1. Name, title and company
2. Age and years in industry (list companies and time with each if more than one)
3. Awards received in industry
4. Book of business
5. Association memberships and positions held within those organizations (if any)
6. 5-7 sentence description of why you, or the person you’re nominating, is deserving of inclusion in EBA’s Top 10 Rising Stars in the benefits business
